Initial and Re-certification fee (3-year certification)*: $500.00**

* additional fees may be charged if the scope of the certification is expanded (i.e. additional sites).

** The Process Verification and Accreditation Office will be increasing the above-noted fee as of January 1, 2012 to $800.00.
